Accident, injury and incident reporting
EMPLOYEES
Form 6A, Worker's report of Injury or Occupational Disease to Employer, is to be completed by employees for work related accidents, injuries or incidents, including near misses, involving College employees (staff, student assistants, faculty and administrators).
Form 6A must be completed by the First Aid Attendant, the Employee, or the Employee's Supervisor and forwarded to Employee Relations within one day of the injury or incident.
STUDENTS ON PRACTICUM
Form 6A, Worker's report of Injury or Occupational Disease to Employer, is to be completed by students on practicum for practicum-work related accidents, injuries or incidents, including near misses, involving College students.
Form 6A must be completed by the First Aid Attendant, the Student, or the Student's Instructor and forwarded to Employee Relations within one day of the injury or incident.
Accident, injury and incident investigations
EMPLOYEES
For accidents, injuries or incidents involving College employees, investigations will be initiated by the Occupational Health and Safety Coordinator or the Manager, Facilities Services.
STUDENTS ON PRACTICUM
For accidents, injuries or incidents involving College students on practicum, the instructor of the student on practicum will complete the investigation.
